Application of multi-slice spiral CT angiography in arteritis

Ma Xiangxing, Zhang Wei, Ma Xiaofeng, Wang Qing, et al. Objective To investigate the clinical value of multi-slice spiral CT (MSCT) angiography in the diagnosis of arteritis. Methods MSCT angiography was performed in 14 patients with clinically diagnosed aortitis. The scan ranged from cervical 2 vertebral level to pubic symphysis. Image post-processing methods included multiplanar reorganization, maximum density projection, volumetric reorganization, endoscopic and vascular. Analysis, fully display the location, extent and extent of the affected blood vessels. RESULTS: CT findings in 14 patients showed different degrees of thickening of the arterial wall, stenosis or occlusion of the lumen. The stenosis of the lumen was characterized by centripetality and partial expansion with stenosis. According to the lumen, there were 8 cases of stenosis-blocking type and 6 cases of mixed type. The reconstructed image combined with the axial image observation can clearly show the degree of stenosis or occlusion of the affected large blood vessels and their branches and the formation of collateral vessels. According to the Lupi-Herrea classification, 5 cases were type I, 2 cases were type II, 6 cases were type III, and 1 case was type IV. The CT findings were consistent with their clinical manifestations. Conclusion MSCT angiography also shows the luminal condition and wall changes of aortitis. The latter is of great significance for early diagnosis and treatment and improvement of prognosis. It can be used as the first choice for this disease. Theintegrated information including recon Structural and transverse images clearly presented various luminalabnormalities, such as stenosis , occlusion and dilatation in the affected aorta, as well as its major branchesand collateral circulation. further, it depicted mural abnormities, especially concentric arterial wallthickening. Steno-occlusive type ( 8 /14 ) was more common than mixed type ( 6 /14) . According to Lupi-Herrea's classtype IV in 1 were recorded in the present study. Conclusion MSCT angiography is reliable imaging modality for the detection of Takayasu'sarteritis. It is useful for early Diagnosis because it allows evaluation of wall thickness rather than immediate theluminal diameter, which is especially important for prognosis.
